Spring Creek SNF “Respite” program provides opportunity for caregivers and family members who need a break from the responsibility of caring for a loved one. Whether traveling out of town or just needing time off to recharge, you can get the rest you need knowing your loved one is in a safe and caring environment. We will provide your loved one with 24-hour nursing assistance with all activities of daily living, medication management, social activities, and therapy services as needed.
Below is a list of required items needed for a Respite Stay:
- Recent history and physical from primary physician
- List of prescribed medications from physician
- Medications in original packaging
- Clothing and personal care items the patient may want (labeled with name)
Respite care may be covered by Private Pay, a Respite Grant through Senior Services of Will County, Hospice and in certain circumstances Medicaid.
